Intraoral Buccal Fat Pad Excision for Lower Facial Contouring: A Case Report

Özet

Objective: The buccal fat pad (Bichat’s fat pad) is a deep facial fat compartment contributing to midand

lower-face volume. Intraoral buccal fat pad excision is a minimally invasive aesthetic procedure

performed to enhance lower facial contour.

Case: A 28-year-old female patient presented with complaints of bilateral cheek fullness. She had no

systemic disease. Clinical examination revealed prominent lower facial fullness. An intraoral approach

was performed under local anesthesia. A 1–1.5 cm mucosal incision was made opposite the maxillary

second molar region. Blunt dissection through the buccinator muscle fibers exposed the buccal fat

pad. Controlled and conservative bilateral excision was carried out. Hemostasis was achieved and the

incision was closed with resorbable sutures. No postoperative complications occurred.

Conclusion: Intraoral buccal fat pad excision is a safe and predictable lower facial contouring procedure

when performed with proper patient selection and meticulous surgical technique.
