Clear Aligner Therapy for Class II Malocclusion: Treatment Strategies and Clinical Considerations- A Narrative Review

Clear aligner therapy has become an important treatment modality in contemporary orthodontics because of its esthetic advantages, improved patient comfort, and continuous advances in digital technology. Although initially indicated for relatively simple malocclusions, its application has expanded to the management of selected Class II malocclusions. Nevertheless, the predictability and effectiveness of clear aligners for sagittal correction remain subjects of ongoing debate. This narrative review aims to provide a comprehensive overview of current treatment strategies for Class II malocclusion using clear aligners, with particular emphasis on clinical indications, treatment protocols, adjunctive mechanics, treatment effectiveness, predictability, and current limitations. The available literature indicates that successful outcomes can be achieved in appropriately selected patients, particularly when clear aligners are combined with auxiliary mechanics such as intermaxillary elastics and temporary anchorage devices. However, treatment predictability decreases as case complexity increases, particularly for complex tooth movements such as root torque, bodily tooth movement, and vertical tooth movements. Careful patient selection, individualized treatment planning, and realistic treatment objectives remain fundamental for successful clinical outcomes. Further high-quality clinical studies are needed to strengthen the current evidence and optimize treatment protocols for Class II malocclusion.
