Stresle ilişkili Nekrotizan Ülseratif Gingivitis ve Tedavisi: 6 Ay Takipli Bir Olgu Sunumu

Özet

Giriş:
   Nekrotizan ülseratif gingivitis (NUG), spiroket ağırlıklı biyofilmin neden olduğu, bulaşıcı olmayan hızlı doku kayıplarının görüldüğü dişeti iltihabıdır. Dişeti papillerinde nekroz, ülserasyon, ağrı ve kanama ile karakterize olup ağız kokusu, lenfadenopati gibi semptomlar eşlik eder. Stres, azalmış konak direnci, yetersiz beslenme, sigara, alkol gibi faktörler NUG’a sebep olabilir. Tedavideki amaç sadece hasta ağrısını azaltmak değil, hastanın periodontal sağlığını geri kazandırmaktır. Bu olgu sunumunda, NUG’dan dolayı şiddetli ağrı ve estetik şikâyeti olan vakanın tedavisi anlatılmıştır.

Materyal&Yöntem:
   23 yaşındaki erkek hasta şiddetli dişeti ağrısı, kanama ve ağız kokusu şikayetleriyle kliniğimize başvurdu. Anamnezinde hastanın stresli bir dönem geçirdiği, sigara içtiği ve ağız hijyeninin kötü olduğu öğrenildi. İlk seansta hastaya, akut iltihabın ve nekrotik yüzeylerdeki pseudo-membranların giderilmesi için hidrojen peroksit emdirilmiş pamuk peletlerle debridman yapıldı. Klinik muayenesinde lenfadenopati saptandığından dolayı kombine antibiyotik, analjezik ve seyreltilmiş hidrojen peroksit gargara reçete edildi. İki gün sonra supragingival diş yüzeyi temizliği yapıldı. Oral hijyen eğitimi verildi, yumuşak diş fırçası kullanımı önerildi. Akut faz kontrol altına alındıktan sonra üçüncü seans lokal anestezi altında kök yüzeyi düzleştirmesi yapıldı.
İki ay sonra iyileşme sonrası düzensizliklerin giderilmesi için hastaya cerrahi gingivektomi yapıldı. Hastanın postop ikinci hafta,1.ay, 3.ay ve 6.ay kontrolleri yapıldı. Takip dönemlerinde enflamasyonun giderildiği ve gingival papillerdeki nekrotik görünümün iyileştiği gözlemlendi.

Sonuç:
  
NUG hastalarının tedavi edilebilmesi ve nüks riskinin en aza indirilebilmesi için kontrol seanslarında etiyolojik faktörler elimine edilmeli ve uygun ağız hijyeni sağlanmalıdır. Bu olguda hastanın 6 aylık takibi sonunda periodontal sağlığın elde edildiği, gingival papillerdeki nekrozdan kaynaklı kaybın iyileştiği görülmüştür. Periodontal tedavileri tamamlandıktan sonra hastalığa bağlı oluşan estetik olmayan dişeti görünümü gingivektomi ile düzeltilebilmektedir.

NUG'un prevalansı düşük olmasına rağmen, oral biyofilme verilen en ciddi yanıtlardan biri olarak önemi göz ardı edilmemelidir. NUG’da nekrotizan periodontitisden farklı olarak lezyonlar diş etiyle sınırlıdır, kemik ve ataşman kaybı yoktur. NUG’un tam olarak tedavi edilebilmesi ve nüks riskinin en aza indirilebilmesi için etiyolojik faktörler elimine edilmelidir. Düzenli takip ve uygun ağız hijyeni, hastalığın tekrarlamasını önlemek için kritik öneme sahiptir.

Introduction:Gingival recession is characterized by the exposure of the root surface and increased sensitivity.The selection of an appropriate mucogingival surgical procedure depends on the extent of the recession and local anatomical conditions .The combination of a laterally positioned flap (LPF) and connective tissue graft (CTG) is one of the approaches used in the treatment of gingival recessions.This case report presents the treatment of patient with Miller Class II gingival recession in the mandibular anterior region using laterally positioned flap and a connective tissue graft. Materials & Methods:36-year-old female patient with no systemic diseases presented to our clinic with complaints of gingival recession and sensitivity. Clinical examination revealed a localized Miller Class II gingival recession measuring 4 mm on tooth #41, accompanied by sensitivity. Due to inadequate keratinized gingiva, thin periodontal phenotype, and shallow vestibule, LPF and CTG were planned. Following the recording of periodontal parameters and intraoral photographs, initial periodontal therapy was performed, and oral hygiene instructions were provided.vertical and oblique incision was made from the distal aspect of the affected region to raise a partial-thickness flap of sufficient width to cover the recession area. The distal margin of the recession site was de-epithelialized. Graft harvested from the patient's palatal region was adapted to the recipient site and secured to cover the root surface. The mobilized partial-thickness flap was laterally positioned to completely cover the connective tissue graft and was stabilized using 6-0 resorbable sutures with the sling technique. Postoperative care instructions were provided to the patient. Sutures were removed at the 2 week postoperative follow-up, and clinical evaluations were performed at 1, 3, and 6 months. Results:This case demonstrated an increase in the amount of keratinized gingiva and a significant improvement in vestibular depth. At the 6-month follow-up, complete root coverage was achieved, and the gained tissue remained stable throughout the observation period. These findings suggest that the combination of LPF and CTG is an effective treatment approach for Miller Class II gingival recessions. Discussion:The laterally positioned flap is an effective technique for increasing the amount of keratinized gingiva and achieving root coverage.The combination of LPF and CTG offers an alternative to the coronally advanced flap, particularly in cases with shallow vestibular depth.The addition of a connective tissue graft enhances the periodontal biotype, leading to more predictable root coverage and improved esthetic outcomes.Various techniques, such as free gingival grafts and acellular dermal matrices, can also be utilized to increase the width of keratinized gingiva and vestibular depth.
