AMAÇ: Çürük dişler, eski ya da kırılmış kompozit restorasyonlarda oluşan renklenmeler estetik olmayan görüntülere sebep olabilmektedir. Çürük doku, kırılmış ve renklenmiş eski restorasyonlar uzaklaştırıldıktan sonra uygulanan kompozit rezinler ön bölgede estetik bir görüntü sağlayabilmek  için konservatif ve düşük maliyetli bir tedavi seçeneğidir.

 

OLGU 1: 38 yaşındaki kadın hasta 11-21 numaralı dişlerindeki restorasyonların kırılması şikayeti ile kliniğimize başvurdu. Yapılan klinik ve radyografik muayenede dişlerde restorasyon kırıklarına ve sekonder çürüklere rastlandı. Hastanın periodontal tedavisi tamamlandıktan sonra 11 -21 nolu dişlerindeki restorasyonlar uzaklaştırılarak kompozit ile yeniden restorasyonuna karar verildi. Hastada izolasyon sağlandıktan sonra mine yüzeyleri %37’lik ortofosforik asit (Scotchbond Universal Etchant, 3M ESPE, ABD) 15 sn uygulandı ve yıkandı. Diş yüzeylerine iki aşamalı self-etch  adeziv ajan (Clearfil SE Bond, Kuraray, Japonya) uygulandı ve kompozit (3M Espe A2 dentin, GC Solare A2 universal, Japonya) ile restore edildi. Aşındırıcı diskler (Sof-lex,3M ESPE, ABD) ve polisaj lastikleri (Sof-Lex, 3M ESPE) kullanılarak bitim ve polisaj işlemleri gerçekleştirildi.

OLGU 2: 19 yaşındaki kadın hastanın 21-22 numaralı dişlerine derin çürükler nedeniyle endodontik tedavi uygulandı ve estetik restorasyon için kliniğimize yönlendirildi. Yapılan klinik ve radyografik muayene sonrası 21-22 numaralı dişlerine direkt kompozit  restorasyon yapılması planlandı. Hastada izolasyon sağlandıktan sonra %37’lik ortofosforik asitle (Scotchbond Universal Etchant, 3M ESPE, ABD) mine 15 sn. ve dentin 10 sn. asitlendi. Su spreyi ile 15 sn yıkanıp hava ile kurutuldu. . Asit uygulanmış diş yüzeylerine iki aşamalı self-etch adeziv ajan (Clearfil SE Bond, Kuraray, Japonya) uygulandı. Daha sonra kompozit rezin (3M Espe A2 dentin, GC Solare X A2 universal) ile restore edildi. Aşındırıcı diskler (Sof-Lex,3m ESPE) ve polisaj lastikleri (Sof-Lex, 3M ESPE) kullanılarak bitim ve polisaj işlemleri gerçekleştirildi.

 

SONUÇ: Ön bölge estetik problemlerin tedavisinde kullanılan kompozit rezinler ile başarılı bir sonuca  tek seansta ulaşılabilirken, konservatif ve düşük maliyetleri ile direkt kompozit rezinler  indirekt restorasyonlara alternatif olabilmektedirler.

 

RESTORATION OF ANTERIOR TEETH THAT HAVE LOSS OF SUBSTANCE WITH COMPOSITE APPLICATION: PRESENTATION OF 2 CASES

OBJECTIVE: Discoloration and deformations in old or broken anterior composite restorations may cause non-aesthetic images. Direct composite resin restorations are a conservative and cost-effective treatment option to provide an aesthetic appearance in the anterior region after removal of broken and colored old restorations.

CASE 1: A 38-year-old female patient was admitted to our clinic with the complaint that the restorations between the teeth in the maxillary anterior region were broken and appear poor. Secondary caries and filling fractures were found in the teeth in the clinical and radiographic examination. After the periodontal treatment of the patient was completed, it was decided to remove the restorations on the teeth 11, 21 and restore them with composite. After isolation, enamel surfaces were acidified with 37% orthophosphoric acid (Scotchbond Universal Etchant, 3M ESPE) for 15 sec. It was washed with water spray for 15 seconds and air dried. The two-stage self-etch adhesive agent (Clearfil SE Bond, Kuraray, Japan) was applied to the acid treated tooth surfaces. Composite resin (3M Espe A2 dentin, GC Solare X A2 universal) was then applied. Finishing and polishing were performed using abrasive discs (Sof-lex, 3M ESPE) and polishing tires (Sof-Lex, 3M ESPE).

CASE 2: A 19-year-old female patient underwent endodontic treatment for maxillary anterior teeth 21-22 with profound caries. After the clinical and radiographic examination, temporary filling materials in teeth 21-22 were removed and replaced by direct composite restoration. After isolation, the patient was treated with 37% orthophosphoric acid (Scotchbond Universal Etchant, 3M ESPE) for 15 sec. and dentine 10 sec. Acidified. It was washed with water spray for 15 seconds and air dried. . The two-stage self-etch adhesive agent (Clearfil SE Bond, Kuraray, Japan) was applied to the acid treated tooth surfaces. Composite resin (3M Espe A2 dentin, GC Solare X A2 universal) was then applied. Finishing and polishing were performed using abrasive discs (Sof-Lex, 3m ESPE) and polishing tires (Sof-Lex, 3M ESPE).

CONCLUSION: While direct composite resins used in the treatment of anterior region aesthetic problems can achieve successful and aesthetic results in a fast and single session, direct composite resins can be an alternative to indirect restorations with conservative and low costs.
