Fokal Epitelyal Hiperplazili Hastanın Nötral Zon Tekniği Kullanılarak Protetik Rehabilitasyonu: Olgu Sunumu

Amaç: Fokal epitelyal hiperplazi (Heck hastalığı), human papilloma virüslerin (HPV) yol açtığı, esas olarak Oral, nadiren de geni-

tal ve anal mukozayı tutabilen, az görülen, kronik bir deri hastalığıdır. Asemptomatik, mukoza ile aynı veya daha beyaz renkte,

1-4 milimetrelik yumuşak papüller şeklinde ortaya çıkar. Bu olgu sunumu, 45 yaşında, Heck hastalığına sahip kadın hastanın nöt-

ral zon tekniği ile protetik rehabilitasyon prosedürünü anlatmayı amaçlamaktadır.

Olgu: 45 yaşında, fokal epitelyal hiperplazili, bimaksiller total dişsiz kadın hasta estetik ve fonksiyonel rehabilitasyon talebi ile

kliniğimize başvurdu. Yapılan muayenede, oral mukozada yaygın ve dağınık yerleşimli mukoza renginde papüller saptandı. is-

keletsel sınıf 3 malokluzyonu olan hastanın üst çenesinde mesnetsiz kret varlığı tespit edildi. Ölçüler alındıktan sonra, üst çeneye

kaide plağı ve mum duvar hazırlandı ve alt kaide plağı üzerine de Viscogel (Dentsply, Germany) materyali uygulandı. Dudak, dil

ve yanakların fonksiyonel hareketleri yaptırılarak nötral zon tespit edildi. Viscogel kaydının negatifini elde etmek için alçıdan

anahtar-kilit şeklinde bir indeks hazırlandı ve oluşturulan negatif boşluğa erimiş mum akıtıldı. Elde edilen alt mum duvarın et-

rafında kondensasyon tipi silikon materyali kullanılarak bir indeks oluşturuldu. Yeni mum duvar kaydına göre diş dizimi yapıldı.

Sonuç: Nötral zon tekniği ile protezin çevre kasların fonksiyonel hareketleriyle uyum içinde olması sağlanarak, tutuculuğu ve sta-

bilitesi daha yüksek protezler elde edilir. Heck hastalığında da olduğu gibi oral mukoza yüzeyinin düzensiz olduğu vakalarda,

nötral zon tekniği kullanılarak fonksiyonel açıdan tatmin edici sonuçlara ulaşılabilir.

Introduction: Focal Epithelial Hyperplasia (Heck's disease) is a rare, chronic disease caused by Human Papilloma Virus (HPV)

which can mostly affect the oral, rarely genital and anal mucosa. It appears like 1-4 mm in diameter, asymptomatic papules which

can be the same color with the oral mucosa or be whiter. This case report aims to describe of the treatment procedure of re-

habilitation of the patient with Heck's disease with neutral zone technique.

Case: 45 year old, female patient who has bimaxillary total edentulism, was applied to our clinic to compensate the esthetic and

functional demands. In oral examination, diffuse and scattered papules with same color as the mucosa were detected. Also

flabby ridges was detected on maxilla of the patient who has skeletal Class Ill malocclusion. After taking impressions, wax-rim

was prepared for upper record baseand Viscogel (Dentsply, Germany) was applied on the lower record base. Functional mo-

vements of the lips, tongue and cheeks were recorded on Viscogel to determine the neutral zone. To obtain the negative im-

pression of the Viscogel record, an key-lock index was prepared from dental stone material and melted wax was poured to the

negative neutral zone area. An index was prepared from condensation silicon impression material around of new wax-rim re-

cord. In guidance of new wax-rim record, denture teeth set-up was made.

Summary: By using the neutral zone technique, prosthesis with higher retention and stability is achieved by ensuring that the

prosthesis is in harmony with the functional movements of the surrounding muscles. In cases where the oral mucosal surface

is irregular, as in Heck's disease, it is possible to achieve satisfactory functional results using the neutral zone technique.
