Pyoderma gangrenosum is a neutrophilic dermatosis which is characterized by chronic, non-infectious ulcers. There is no standard treatment or algorithm for pyoderma gangrenosum. The choice of treatment varies depending on the type of ulcer, growth rate of ulcer, general condition of the patient, and the underlying disease. Thrombocyte-rich fibrin which contains platelets and leukocytes is an autologous membrane. Through this feature, it is used to repair soft tissue defects. In this case, a 56-year-old female patient with pyoderma gangrenosum who did not see any benefit from systemic therapies is presented for having significant improvement on her ulcer after thrombocyte-rich fibrin application. The case is presented in order to emphasize that the platelet-rich fibrin therapy is easy to be prepared and administered and also it is autologous and inexpensive at the same time.
