Introduction: In this case report, the treatment of dental malocclusion with a removable
appliance is presented in a patient with a narrow maxilla and dental crowding in the mandibula.
Case Report: An 8-year-old, systemically healthy girl in the mixed dentition period applied to
İnönü University Faculty of Dentistry, Department of Pedodontics with the complaint of the
appearance of her upper anterior teeth. In the clinical examination, severe crowding and
narrowing in maxilla; and mesialization of the left permanent first molar tooth in mandibular,
due to early extraction of the primary molar tooth, were detected. The slow expansion appliance
with screw to the upper jaw was made to distalize the permanent 1st molar tooth (36) to the
lower jaw with the occlusion of the distalization apparatus raised. The patient was
recommended to use the appliance for at least 16 hours a day. The patient's parent was told to
turn the expansion screw once in 3 days. It took 2 months to achieve the ideal dental
relationship. After the expansion and distalization treatment was completed, the band loop was
planned because tooth numbered 54 was lost early in the upper jaw, and the lingual arch space
maintainer because teeth 75, 84, 85 were lost in the lower jaw. The follow-up of the patient
continued during the transition from mixed dentition to permanent dentition.
Conclusion: In this case presented, the patient, who had crowding in the mixed dentition
period, gained space in the lower and upper jaw with the modified removable appliance. With
this treatment, which provides early results and does not force the patient, fixed orthodontic
treatment need in the future is eliminated
