Bir Ankiloglossi Olgusunun Frenektomi ile Tedavisi ve 6 Aylık Takibi

Özet

Amaç: Lingual frenulum, bir tarafta dilin ventral yüzeyini ağız tabanına, diğer tarafta mandibulanın bazal kemiğine bağlayan mukozal dokudan oluşan membranöz bir ligamenttir. Bazı bireylerde frenulumun lifleri dilin ucuna bağlanarak fizyolojik hareketlerini kısıtlar. Ankiloglossi veya dil bağı, kısa lingual frenulumun veya gergin genioglossus kasının dil hareketini kısıtladığı konjenital bir malformasyondur. Yeni doğan bebeklerde görülme sıklığı %4'e kadar çıkabilmektedir. Lingual frenulumun cerrahi olarak serbestleştirilmesi (frenotomi/frenektomi), doğumdan başlayıp yetişkinliğe kadar gerçekleştirilebilen, giderek yaygınlaşan bir prosedür haline gelmiştir.

Olgu Sunumu: Sistemik olarak sağlıklı, 16 yaşındaki kadın hasta R,S gibi harfleri telaffuz ederken güçlük yaşaması sebebiyle periodontoloji kliniğine başvurdu. Hastanın klinik muayenesinde kısa ve kalın lingual frenulum varlığı tespit edildi. Faz I periodontal tedavisi tamamlandıktan sonra lingual frenektomi operasyonu planlandı. Lokal anestezi ile bistüri kullanılarak ilgili bölgedeki kas ataşmanları hasta dil hareketlerini kolayca yapabilecek hale gelinceye kadar diseke edildi. Yara kenarları 6/0 rezorbe olabilen sütur ile primer olarak kapatıldı. Cerrahi sonrası iyileşme sorunsuz gerçekleşti. Hastaya iyileşme döneminde konuşma egzersizleri yapması, sesli kitap okuması tavsiye edildi. Hasta takip randevularına geldiğinde 1., 3. ve 6. ay kontrollerinde dilin fonksiyon halinde olduğu, konuşma probleminin tatmin edici seviyede düzeldiği görüldü.

Sonuç: Bu olgu sunumu, lingual frenektomi operasyonunun ankiloglossiye bağlı konuşma güçlüğünün çözümünde etkili ve başarılı bir cerrahi teknik olduğunu göstermektedir. Operasyon sonrası iyileşme sürecinin sorunsuz ilerlemesi ve hastanın konuşma problemlerinde belirgin bir düzelme sağlanması, lingual frenektominin hem kısa hem de uzun vadede tatmin edici sonuçlar sunduğunu ortaya koymaktadır.

Aim: The lingual frenulum is a membranous ligament composed of mucosal tissue that connects the ventral surface of the tongue to the floor of the mouth on one side and to the basal bone of the mandible on the other side. In some individuals, the fibers of the frenulum attach to the tip of the tongue, restricting its physiological movements. Ankyloglossia, or tongue-tie, is a congenital malformation where a short lingual frenulum or a tense genioglossus muscle restricts tongue movement. The prevalence of ankyloglossia can be as high as 4% in newborns. The surgical release of the lingual frenulum (frenotomy/frenectomy) has become an increasingly common procedure that can be performed from birth through adulthood.

Case Report: A 16-year-old female patient, who was systemically healthy, presented to the periodontology clinic with difficulty pronouncing letters such as R and S. Clinical examination revealed a short and thick lingual frenulum. After completing Phase I periodontal treatment, a lingual frenectomy operation was planned. Under local anesthesia, the muscle attachments in the relevant area were dissected using a scalpel until the patient could easily perform tongue movements. The wound edges were primarily closed with 6/0 absorbable sutures. Postoperative healing was uneventful. The patient was advised to perform speech exercises and read aloud during the healing period. At follow-up appointments at 1, 3, and 6 months, it was observed that the tongue was functioning well and that the speech problem had improved to a satisfactory level.

Result: This case report demonstrates that lingual frenectomy is an effective and successful surgical technique for resolving speech difficulties associated with ankyloglossia. The smooth progression of the postoperative healing process and the significant improvement in the patient’s speech problems highlight that lingual frenectomy provides satisfactory results in both the short and long term.
