Özet

A systemically healthy 23-year-old female patient presented with pain in the

right mandibular region. Panoramic radiography and cone-beam computed tomography (CBCT)

examinations revealed a deeply impacted mandibular second molar in close proximity to the inferior

alveolar canal. The buccal lid technique was preferred to minimize bone loss and surgical morbidity.

Following full-thickness mucoperiosteal flap elevation under local anesthesia, the buccal cortical bone

segment was atraumatically removed using piezosurgery. The tooth was removed via odontotomy while

preserving the inferior alveolar nerve. Following debridement, the buccal segment was repositioned to its

anatomical site, fixed with a mini-plate and screw system, and the surgical site was primarily closed. No

neurosensory deficits were observed during postoperative follow-ups.
