Özet
Amaç: “All-on-4” konsepti, vertikal ve horizontal
kret yetersizliği durumunda konvansiyonel implant cerrahisine alternatif olarak
distaldeki implantların açılı yerleştirilerek distal kantilever uzantılı bir
restorasyon ile tam dişsiz çenelerin sabit protetik rehabilitasyonudur. Böylece
kemik augmentasyonu gibi ileri cerrahi işlemler uygulanmadan hastanın estetik
ve fonksiyonel beklentileri en kısa sürede ve en ekonomik şekilde
karşılanabilmektedir. Bu olgu sunumunda, tam dişsiz bir hastanın “All-on-4”
tedavi konsepti ile rehabilitasyonu ve vakanın 3 yıllık takibi rapor
edilmektedir.
Olgu Sunumu: 65 yaşında herhangi
bir sistemik rahatsızlığı olmayan erkek hasta, çiğneme güçlüğü ve hareketli
protezinde retansiyon problemi nedeniyle implant yaptırmak üzere İnönü Üniversitesi
Diş Hekimliği Fakültesi Periodontoloji Kliniğine başvurdu. Klinik ve
radyografik muayene sonucunda ileri derecede kemik rezorpsiyonu olduğu, sağ ve
sol maxiller sinüslerin pnömatize olduğu belirlendi. Konik ışınlı bilgisayarlı
tomografi taraması sonrası maxiller lateral kesici dişler ve maxiller ikinci
küçük azılar bölgesine 4 adet implant yapılması planlandı. İmplant cerrahisi
sırasında üst çene posterior bölgelere 2 adet açılı implant yerleştirildi; alt
çenede ise mandibuler sağ ve sol kanin dişler bölgesine 2 adet implant
yerleştirildi. 12 haftalık osseointegrasyon süreci sonrasında iyileşme
başlıkları takılarak protetik üst yapı tamamlandı.
Sonuç: İlave cerrahi işlemlerden kaçınılarak hastanın
estetik ve fonksiyonel beklentileri karşılanmıştır. Hastanın 3 yıllık izleminde
implant başarısı, protez başarısı, açılı ve düz implantların etrafındaki
marjinal kemik kaybı değerlendirilmiştir. “All-on-4” konsepti aşırı atrofik
çenelerde başarılı ve güvenli bir tedavi yöntemi olarak tercih edilebilir.

Abstract
Aim: “All-on-4” treatment concept is
the fixed prosthetic rehabilitation of edentulous jaws with a distal cantilever
extended restoration by tilting distal implants as an alternative to
conventional implant surgery in case of vertical and horizontal ridge insufficiency.
Thus, the aesthetic and functional expectations of the patient can be met most
economically and rapidly without any advanced surgical procedures such as bone
augmentation. In this case report, the rehabilitation of an edentulous patient
with the “All-on-4” treatment concept and a 3-year follow-up are reported.
Case
Presentation: A 65-year-old, systemically healthy
male patient with complaint of poor chewing ability and retention referred to
Inonu University, Faculty of Dentistry, Department of Periodontology to have
dental implants. During clinical and radiographic examination, it was detected
that there was advanced bone resorption, the right and left maxillary sinuses
were pneumatized. After the examination of the conical beam computed tomography
scan, it was planned to insert 4 implants for maxillary lateral incisors and
maxillary second premolars. During implant surgery, 2 tilted implants were
placed to the posterior regions of the upper jaw; 2 implants were placed for
the mandibular right and left canine teeth. After 12 weeks of osseointegration
period, healing caps were attached and prosthetic superstructure was completed.
Conclusion: The aesthetic and functional expectations of the
patient were met without any additional surgical procedures. The parameters of
implant success, prosthetic success and marginal bone loss around tilted and
straight implants were assessed on 3-year follow-up. “All-on-4” concept can be
preferred as a successful and reliable treatment method for the rehabilitation
of excessive atrophic jaws.
