Miller Sınıf II Dişeti Çekilmesinde Laterale Pozisyone Flep ve Bağ Dokusu ile Tedavisi: Olgu Sunumu

Özet
Giriş:
Dişeti çekilmesi, kök yüzeyinin açığa
çıkması ve hassasiyet oluşması ile karekterizedir. Çekilmenin boyutuna ve lokal
anatomik koşullara bağlı olarak uygun mukogingival cerrahi prosedürün seçilmesi
gerekir. Laterale pozisyone flep (LPF) ve bağ dokusu greftinin(BDG) birlikte
uygulaması dişeti çekilmelerinin tedavisinde kullanılan yaklaşımlardan birisidir.
Bu olgu sunumunda alt ön bölgede Miller sınıf II dişeti çekilmesi olan hastanın
laterale pozisyone flep ve bağ dokusu ile tedavisi sunulmuştur.
Materyal&Yöntem:
36 yaşında sistemik rahatsızlığı olmayan
kadın hasta dişeti çekilmesi ve hassasiyet şikayeti ile kliniğimize başvurdu.
Klinik muayenede 41 numarada 4 mm olarak ölçülen lokalize Miller sınıf II
dişeti çekilmesi ve buna bağlı hassasiyet tespit edildi. Yetersiz keratinize
dişeti,ince dişeti fenotipi ve sığ vestibülden dolayı LPF ve BDG planlandı.
Periodontal kayıtların ve ağız içi fotoğrafların alınmasının ardından, başlangıç
periodontal tedavi gerçekleştirildi ve oral hijyen eğitimi verildi. İlgili
bölgenin distalinden vertikal ve oblik insizyon yapılarak çekilme alanını
örtecek genişlikte yarım kalınlık flep kaldırıldı.Çekilmenin distali
deepitelize edildi. Hastanın üst palatinal bölgesinden alıcı yatağa uygun greft
alınarak kök yüzeyini de örtecek şekilde sabitlendi. Kaldırılan yarım kalınlık
flep serbestleştirilerek bağ dokusunun tamamını örtecek şekilde laterale
kaydırıldı ve 6-0 rezorbe olabilen sütur kullanılarak askı tekniği ile
sabitlendi. Hastaya postoperatif öneriler anlatıldı. Post-op 2. Haftada süturlar
alındı, 1. Ay, 3. Ay ve 6. Ay klinik takipleri yapıldı.
Sonuç:
Bu
vakada keratinize
dişeti miktarının arttığı ve vestibüler derinlikte ciddi bir artış olduğu görülmüştür. 6 aylık takip süresinin sonunda çekilmenin
kapatıldığı ve kazanılan dokunun takip edilen dönemlerde stabilitesini koruduğu
gözlemlenmiştir. Bu vaka BDG ile LPF tekniğinin Miller sınıf II
dişeti çekilmelerinde etkili bir tedavi yöntemi olduğunu göstermektedir.
Tartışma:
Laterale pozisyone flep, keratinize dişeti miktarını artırmak ve kök yüzeyini kapatmak için etkili bir tekniktir. LPF ile BDG’nin kombine uygulaması vestibül derinliğin sığ olduğu durumlarda koronale pozisyone flebe alternatif bir yaklaşım sunar. Bağ dokusu greftiyle kombine edilmesi biyotipi kalınlaştırarak daha öngörülebilir kök yüzeyi kapama ve daha estetik sonuçlar elde edilebilir. Keratinize diş eti genişliğini ve vestibül derinliği artırmak için serbest dişeti grefti, aselüler dermal matriks gibi çeşitli teknikler uygulanabilir.

Introduction:Gingival recession is characterized by the exposure of the root surface and increased sensitivity.The selection of an appropriate mucogingival surgical procedure depends on the extent of the recession and local anatomical conditions .The combination of a laterally positioned flap (LPF) and connective tissue graft (CTG) is one of the approaches used in the treatment of gingival recessions.This case report presents the treatment of patient with Miller Class II gingival recession in the mandibular anterior region using laterally positioned flap and a connective tissue graft.
Materials & Methods:36-year-old female patient with no systemic diseases presented to our clinic with complaints of gingival recession and sensitivity. Clinical examination revealed a localized Miller Class II gingival recession measuring 4 mm on tooth #41, accompanied by sensitivity. Due to inadequate keratinized gingiva, thin periodontal phenotype, and shallow vestibule, LPF and CTG were planned. Following the recording of periodontal parameters and intraoral photographs, initial periodontal therapy was performed, and oral hygiene instructions were provided.vertical and oblique incision was made from the distal aspect of the affected region to raise a partial-thickness flap of sufficient width to cover the recession area. The distal margin of the recession site was de-epithelialized. Graft harvested from the patient's palatal region was adapted to the recipient site and secured to cover the root surface. The mobilized partial-thickness flap was laterally positioned to completely cover the connective tissue graft and was stabilized using 6-0 resorbable sutures with the sling technique. Postoperative care instructions were provided to the patient. Sutures were removed at the 2 week postoperative follow-up, and clinical evaluations were performed at 1, 3, and 6 months.
Results:This case demonstrated an increase in the amount of keratinized gingiva and a significant improvement in vestibular depth. At the 6-month follow-up, complete root coverage was achieved, and the gained tissue remained stable throughout the observation period. These findings suggest that the combination of LPF and CTG is an effective treatment approach for Miller Class II gingival recessions.
Discussion:The laterally positioned flap is an effective technique for increasing the amount of keratinized gingiva and achieving root coverage.The combination of LPF and CTG offers an alternative to the coronally advanced flap, particularly in cases with shallow vestibular depth.The addition of a connective tissue graft enhances the periodontal biotype, leading to more predictable root coverage and improved esthetic outcomes.Various techniques, such as free gingival grafts and acellular dermal matrices, can also be utilized to increase the width of keratinized gingiva and vestibular depth.
