ROOT AND ROOT CANAL MORPHOLOGY OF MANDIBULAR SECOND PERMANENT MOLARS IN THE TURKIYE POPULATION: A CBCT STUDY

Özet

Introduction: Detailed knowledge of mandibular second molar morphology is essential to prevent missed canals and treatment failure. Ethnic and demographic variations may influence root canal configuration patterns.

Objective: To evaluate the root and root canal morphology of mandibular second molars in a Turkish population using cone-beam computed tomography and the Ahmed classification system, and to assess bilateral symmetry and pulp chamber height distribution.

Materials and Methods: A retrospective cross-sectional CBCT study was conducted on 527 mandibular second molars from 289 patients. Canal configurations were classified according to the Ahmed system. Patient age and sex were recorded. Pulp chamber height was measured in millimeters. Bilateral symmetry was assessed in patients with both teeth present.

Results: The most common configurations were M-2-1 D-1 (25.1%), M-2 D-1 (20.9%), and M-1-2-1 D-1 (15.2%). C-shaped canals were identified in 12.7% of teeth (n=67). Bilateral teeth were available in 237 patients, among whom complete symmetry was observed in 75.9% of cases. Bilateral C-shaped configuration was present in 10.1% of patients, while unilateral C-shaped anatomy was observed in 3.8%. Mean pulp chamber height demonstrated a progressive decrease across age groups (2.158 mm in 16–35 years; 1.687 mm in 36–55 years; 1.317 mm in 56–76 years).

Conclusions: Mandibular second molars in this Turkish population exhibited considerable morphological diversity with a high rate of bilateral symmetry. C-shaped configurations were relatively prevalent. Age-related reduction in pulp chamber height may have clinical implications during access cavity preparation.
